$800 deposit or full payment of $1800 due now. We accept Cashier’s Check, Money Order, Credit Card and Venmo @BSE-1942_. Upon registration, if you are only paying your deposit of $800. Your remainder may be paid either by credit card or Venmo. If you are paying your remainder by credit card please call An-g at 385-446-4448. Monday, Wednesday or Thursday between 10am-2pm. I understand that the deposit of $800.00 is a non-refundable deposit once accepted into the Invitational. Furthermore, I understand that this deposit is only to secure my spot and does not cover the team fees to participate.

I understand that if I cancel for any reason Prior to October 15th, I will receive a refund of $800 if and only if my full registration fee has been paid.

I understand that if I cancel for any reason AFTER October 15th, there will be absolutely no refund given.

I understand that payment must be made in the form of Cashier’s Check, Money Order, Credit Card or Venmo@BSE-1942_. Payments should be written out to “Bayside Sports & Entertainment LLC”.

I understand that we must include an Official Team Roster provided by your league, which lists all Player’s Names, Birth Dates,
Weights and Jersey Numbers.
