Online registration is now closed. Please download the registration, complete it and email it to Jamie at Jamie@sunyfl.com. DO NOT send a payment in any form (For example Venmo, Cashier’s Check or Credit Card) until Jamie accepts your registration and authorizes you to complete a payment. If you send in a payment without prior authorization from Jamie, your payment WILL BE sent back to you along with any applicable fees. These fees will be a loss to you.

I understand that the deposit of $800.00 is a non-refundable deposit once accepted into the Invitational. Furthermore, I understand that this deposit is only to secure my spot and does not cover the team fees to participate.

I understand that if I cancel for any reason Prior to October 15th, I will receive a refund of $800 if and only if my full registration fee has been paid.

I understand that if I cancel for any reason AFTER October 15th, there will be absolutely no refund given.

I understand that payment must be made in the form of Cashier’s Check, Money Order, Credit Card or Venmo@BSE-1942_. Payments should be written out to “Bayside Sports & Entertainment LLC”.

I understand that we must include an Official Team Roster provided by your league, which lists all Player’s Names, Birth Dates,
Weights and Jersey Numbers.
